Parents/Guardians,
On February 13th, 2026, MRHS will administer the Illinois Youth Survey to all students. This survey covers drug use, beliefs about drug use, healthy behavior, feelings about school, and family support and rules about substance abuse. This survey collects no identifiers that can be linked to a specific student.
You can opt your student out of the survey by filling out the form via this link: https://iys.cprd.illinois.edu/
conducting/opt-out and emailing it to Eric Matthews at ematthews@mr238.org, or by signing a form for your student and bringing it back to the office. There are English and Spanish copies of the opt out form available in the Main Office. If you plan to opt your student out of this survey, please have the form completed no later than Thursday, February 12th.
