$10 Sports Physicals at MRHS on Thursday

Eagle View Community Health System will be offering Sports Physicals at Monmouth-Roseville High School on Thursday, August 4th from Noon - 4 pm.
**These are just sports physicals. They do not satisfy the 9th grade physical requirement.
Cost: $10
No appointments necessary. Just arrive at the high school main office entrance with $10 and a completed IHSA/IESA Physical Form (fill out the entire front and signatures on the front & back).
Form is linked below. Forms can also be picked up in the office on Wednesday and Thursday as well.
