Sports Physicals offered at MRHS on May 4th

Eagle View Community Health System will be offering Sports Physicals at Monmouth-Roseville High School on Wednesday, May 4th from Noon - 4:30 pm
Cost: $10
Please e-mail Jeremy Adolphson ([email protected]) to reserve a time for your student to receive a physical.
Must bring the form linked below with the front filled out with parent & student signatures on the front & back. **Students can pick up a copy from Coach Adolphson as well.
