Eagle View Community Health System will be offering Sports Physicals at Monmouth Roseville High School on Monday, August 9th from 2-4 pm.
Cost: $10
Must bring the form attached below with the front filled out with parent & student signatures on the front & back.
Registration will be outside the AP Room.
