Eagle View Community Health System will be offering Sports Physicals at Monmouth Roseville High School this Friday from 1:30 - 3:30 pm.
Cost: $10
Must bring the form linked below with front filled out with parent & student signatures on the front & back.

Registration will be in the foyer outside the gym.
